New patient information forms
To save time during your first visit, please fill out the appropriate patient information form. Complete the form as fully as possible, then print it and bring a copy with you to your appointment. You can also print an extra copy for your records.
Insurance
We accept all major credit cards and most major dental insurance carriers. We also accept Medicaid and PeachCare for children.
Care for the whole family
Our office offers a full range of dental services for both adults and children. We love children! Our office is designed with colorful, fun spaces to make dental visits enjoyable for your little ones.
Cancellations
To help us serve you and other patients efficiently, please notify us at least 24 hours in advance if you need to cancel your appointment. Missed appointments may result in a cancellation fee.
